Summary
Family Ways is a partnership with communities to support pregnant, parenting families and children. Services will be provided to 3 specific focus communities: U.S. born Black/African American, the American Indian/Alaskan Native, and the Native Hawaiian/Pacific Islander communities.
The Peer Community Specialists will work in a team environment and practice care coordinated with other disciplines. The team will include a Public Health Nurse, Nutritionist, Social Worker and Administrative Support staff. Peer Community Specialists will be responsible to partner with clients and families in creating a care plan and support them through pregnancy or parenting. Team members will be upheld to Family Ways Guiding Principles: leading with racial equity, respect, participant-centered services with strength-based practice, collaborative decision making, accountability, and continuous improvement in their services and programming.
Competitive candidates should have experience in working closely and collaboratively with individuals and families that are members of one or more of the 3 focus communities, have experience with client interaction in a variety of settings (including home/field visits), and be comfortable accompanying participants and families to appointments. Applicants should have demonstrated effective experience in teaching and coaching, collaboration with others, and strong communication skills that are inclusive and inviting, especially in difficult situations. Competitive candidates will need to have experience in and the ability to participate in and assist with continuous improvement efforts.

These positions are situated in the Community Health Services Division in the Parent Child Health Program. Staff may report to work at one or more community-based organizations or county offices as part of their work locations.
Job Duties
Direct care and coordination
Partner with program participants and families to develop care plans that may include:
Identifying social and health systems resources
Identifying and connecting participants to community resources
Conduct educational and informational learning opportunities for staff, clients, community groups and other professionals.
Respond to calls, emails, or requests in a timely manner.
Promote and/or market sound, accurate, culturally appropriate educational materials, and information.
Collaborate with program participants, families, communities, and King County programs to support access to services and referrals.
Foster and coordinate community and agency partnerships.
Attend community events.
Help assess needs for education for program participants, families, and other program staff.
Assist with identifying program and curriculum development needs.
Implement and evaluate education curriculum.
Arrange for culturally relevant and linguistically appropriate presentations for specific audiences.
Implement processes to ensure public and community involvement.
Maintain and update a library of training and reference materials.
Gather and assemble data.
Perform other duties as assigned.
Partnership and Community Engagement
Foster relationships with community organizations
Support community engagement activities
Support expanding partnerships and program participation.
Collaborate and coordinate with other King County programs and community-based organizations to advance racial equity and social justice. This may include active participation in development of racial equity/anti-racism trainings or curriculum.
Working as a Peer Community Specialist
Engage actively in program goals/objectives.
Be an active partner in participant-centered, strengths-based practices.
Apply trauma recovery and healing strategies to program participants.
Prioritize racial equity and social justice frameworks and principles in collaborative work with program team and with other County programs.
Work collaboratively with evaluation team and electronic medical record support staff.
Collaborate and share decision making with team members fostering and creating space for authentic and respectful dialogue.

Experience, Qualifications, Knowledge, Skills
Knowledge of:
theories and practices in health promotion

Skills in effectively communicating:
Information to a wide variety of audiences, making individual or group presentations
Interpersonal communications skills and ability to interact professionally with culturally diverse individuals during a time of crisis and distress.
Oral and written communications skills
Skills and experience using computer, Microsoft programs (Office 365, Word, Outlook, Teams, etc.), electronic health records, web-enabled tools, video meeting platforms and a wide range of audio-visual equipment and programs. Candidates must be proficient and comfortable with computer use to perform the essential functions of this position.
Customer service, health care, or education skills or experience.
Excellent organizational skills, with attention to detail and accuracy.

Experience as Community Health Worker, Navigator, or Community Organizer.
