Jim has never been diagnosied with any medical issue but he drinks a lot. He even sustainred bruieses from his seat belt and a concussion from the wreck while he was drunk. He however, doesn’t believe he is depressed.

Family

Jim is a a divorcee. He also has a son. He seperated with his wife because of infidelity issues which led to custody battles between him and his wife. Jim also comes from a divorced family. his father drank a lot after his divorce but he underwent counseling that made him sober.

Substances Abuse

Jim used to drink even before his divorce but he did so occasionally. After his divorce, his drinking habits have increased drastically that he is even considered an alcoholic. When he starts to drinks, he drinks until he passes out and he is at the risk of losing his custodial rights. He admits from moving just an occasional drinker to a daily drinker. However, he doesn’t drink while with his son.

Developmental and Social

Jim works at a construction. He is divorced and this was due to infidelity issues. He risks loosing his license and was forced to pay the retainer in order to keeep the licence.

The clients level of drinking has increased drastically since he was divorced. He has moved from an occasional drinker to a day to day drinker making him an alcoholic.

The client divorced with his wife on grounds of infidelity.

Relapses number and Severity

The client drinks until he passes out. He has sustained injuries and concussion due to alcoholism. He has even completely lost his custodial rights.

He was forced to divorce his wife and he even doest have custodial rights over his son.

Guiding and counselling

Guiding and counseling on the importance of remaining faithful to one partner.

Objectives and Goals

Long and short term goals and objective towards achievemmnt of the goal

Long-Term Goals

Stop taking alcohol

Gain self control on issues of infidelity

Short-Term Goals

Attend guiding and counseling sessions

Make the client accept his past that he is divorced and now he needs to move on with his life

Objectives

The main objerctive with Jim is to make him realize that his life is important and he has to stop taking alcohol and mocve n with his life. The other objective is creating a supportive environment so that he cant relapse to alcoholism.

Termination and Discharge Plans

Discharge Plan

Jim will only be discharged if he completes his 6 month counseling sessions. It is recommended that he doesn’t associate with alcoholics or work where alcohol is easily available.s

Termination Plan

The client will be terminated only when he changes his behaviors. He should completely stop taking drugs and also show he has completerly refrained from infifedility. He should show consistent progress and the therapists and counselors should ensure he has completely rehabilitated.
