Implementation of a collaborative nursing practice in the delivery of healthcare services with respect to nurse leadership in management of diabetes.

The delivery of care in which nurses and other healthcare professionals work collaboratively to accomplish a shared clinical objective is known as a collaborative nursing practice. Working together as a team in a collaborative nursing practice allows healthcare providers to pool their resources, knowledge, skills, and experience in order to better serve their patients. This, in turn, results in a higher standard of care for everyone involved. As a result, patient outcomes will improve because of better treatment and better patient outcomes. The plan will include various parties including nurse leaders, nursing professionals, diabetes patients and family members.

Providing high-quality health care services and ensuring that patient outcomes are improved are the fundamental objectives of nursing. To improve the quality of care provided in health care facilities, it is necessary to use a variety of various ways. Promoting medical expert collaboration and enacting regulations that encourage it are two more options. “Collaboration” is a term used to describe when several specializations and departments work together to reach a therapeutic goal. Cooperation within the nursing profession is crucial because it facilitates communication and collaboration across departmental specialists from throughout the healthcare system. One may see the enormous impact of collaborative nursing practice immediately. With cooperation across multiple departments, it is much simpler to operate as a team, giving team members the chance to share knowledge and experience from diverse knowledge bases to learn from one another. Nurses cooperate and support one other when they study and share information in a group setting. The topic of the project is: Implementation of a collaborative nursing practice in the delivery of healthcare services with respect to nurse leadership in management of diabetes.

Purpose Statement

The purpose of this project is to explore the implementation of a collaborative nursing practice in the delivery of healthcare services. The main aim of the nursing practice is to ensure that the quality of healthcare services being provided to patients is of the best quality, it is promoting the safety of the patients, and it is improving the patient outcomes in the healthcare facilities. The project will help in ensuring that the delivery of care is through a collaborative practice one that employs professionals from different nursing disciplines working together in teams. This will help in ensuring that the services being provided are patient-centered, thus improving the quality of care among diabetes patients.

Site and service population

The project will be carried out in a managed care organization and the population of interest is the nurse leaders and nursing professionals in the management of diabetes.

Three objectives that are measurable, realistic, and directly relate to the proposed practice contract in an area of patient care, health care organization leadership, or research.

Reducing the readmission rate of patients in the healthcare facility: When the nursing professionals work together in a collaborative nursing environment, they can improve the quality of care through the provision of patient-centered services that improve the patient outcomes. This objective will be measured through the evaluation of the readmission rate of patients.

Decreasing the cost of healthcare services: Having a team of nurses working together reduces the risk of mistakes in patient care, which in turn reduces the cost of providing care.

Increasing patient participation in their care plan: Sharing experience and information among team members in a collaborative nursing practice streamlines the interdisciplinary process which reduces multiple member contacts from multiple members of the team at different times and increases member involvement in their care.

Strategies–brief discussion of strategies to meet objectives, including potential internal and/or external resources and methodology for intervention and evaluation.

To ensure that the objectives are met, the project will involve various strategies:

Establishment of details of the project as well as the goals of the project.

Analysis of the project and the requirements of the team involved in the project.

Ensure that there is open communication among the parties involved.

Measure and ensure that performance is monitored throughout the project.

Avoid any project scope deviation.

Ensure that there is proper management and evaluation of project risks.

Action plan–brief discussion of action steps to accomplish objectives and proposed timeline.

To ensure that the objectives are accomplished, the following action plan will be utilized:

Team creation. There will be a creation of a nursing team that will attend to patients in a collaborative nursing environment.

Leadership. The team will be led by a nurse leader who will ensure that the team is focused and motivated towards the achievement of the project objectives.

Team membership. The team will comprise nurses from various disciplines who will share their ideas and expertise towards the achievement of the objectives.

Measurement of the objectives. The objectives will be measured through the analysis of the number of the patients assessed using the collaborative nursing practice that have been readmitted and compared to those assessed through the normal treatment process. This will be the same for the cost of treatment as well as the member involvement for diabetes patients during the treatment process using the collaborative nursing practice compared to the usual treatment process.

Timeline. The monitoring of the process will be over a period of 4 weeks.
