Quality Improvement Analysis Tool.

Introduction

Each health care system is established on a complex network of care pathways and processes. The care quality delivered by the system depends to a greater extent on how efficient this network operates, and how best the individuals who manage and provide care work together. The objective of quality improvement is to provide high standard quality care to patients and to elevate a nation’s population health. Quality improvement entails giving the people who are closest to the problems and issues affecting the care, quality time, skills, permission and resources they may need to find the solution (Glouberman 2018).

Quality of data influence the usability of the data in HER systems.Genearally, the data should always be relevant and accurate and those who collect data must validate its quality. This is because accuracy is one of the important aspect of block chain interoperability thus quality improvement of this application must be tested with quality improvement analysis tool to confirm its efficiency especially in helping clinician with care plans for re hospitalized diabetic, substance abusers and mental health patients.

Quality Improvement Method.

The electronic health record interoperability influence most related health information technology. The four major domain functionalities in the EHR systems that assists the clinicians in decision making include; patient demographic data records and clinical data, managing and viewing of laboratory tests results and data imaging, management of electronic prescription and order entry and lastly supporting clinical decisions that includes drug contraindications warnings and drug interaction of the hospitalized diabetic and substance abusers.

The problem of block chain interoperability application will be identified when tested using quality improvement tools and majorly in the study the tool will be the ISO 9000 quality improvement tool. The examination will aim to establish any significant relationship between ISO 9000 principles that is discussed below and the bock chain EHR interoperability .The emphasis will be on the management quality in the block chain interoperability application implementation and if the use would lead to improvement of the block chain interoperability application in assisting the clinicians in developing after care plan for the re hospitalized diabetic ,substance abusers and mental health patients. Below are the .ISO 9000 testing principles in relation to the block chain interoperability applications.

Understanding the problem.

Before engaging on how to handle the problem, it is prudent to understand the why and how the problem rose. Sparing time to do so by analyzing variety of data in partnership with patients and staff, shall aid in finding the root cause of the problem than solving the symptoms. For instance in block chain interoperability data inconsistency of patients from the facilities was witnessed thus it is very critical to understand the root cause of the same (Varkey 2016).Through application of cause and effect tool in problem identification that is always designed to enable players to identify nearly all the potential causes not just the most obvious ones. Immediately after the identification of the application default such as data inconsistency , members can now employ various tools for further investigation such as process mapping, survey and patient interviews. Process mapping tool is used to verify every process step. It is majorly used to map the journey or pathway in part or all the patient supporting processes and health care journey (Kumar 2017).Process mapping tool is always useful in staff engagement in fathoming how various steps of the system fits each other, the valuable steps and where delays or waste be spotted. Patients journey mapping in this regard the re hospitalized diabetes, mental health and substance abuse patients which involve multiple healthcare providers is essential in identity of any quality challenges which occur at the interludes between organizations and teams.

Designing improvement.

It is very essential to give convenient time when designing an improvement intervention and the delivery period. The development interoperability block chain application requires specific objective and measurable and clear targets .This could aid in resources and support attraction from managers and leaders for a smooth implantation of interoperability application to assist the patients. It is also very essential to identify any subsequent challenges which would require to be addressed in order to achieve the efficiency of the application. A driver diagram captures the significant issues and identify plans required to tackle them. A logical model that spells out a theory of change on how an intervention should operate is a another needful tool. If identifying an intervention, it is important to review how other application had been improved .When this is done ,it reduces the risk of reinventing the wheel by repetition of a similar work which could have already been done somewhere (Silver 2015)..

Data and measurement improvement

Gathering and measurement of data are key elements in any attempt to elevate quality or performance and are required in an assessment of the set aims against the impact. When attempting to assess the impact of change to a more complex system such as the block chain interoperability, an integration of measures is used more often like outcome and process measures. The measure for improvement targets identifying the occurrence changes while the intervention itself is under test for it to be refined over a duration in response to the available data of the re hospitalized diabetic, substance abusers .In change measuring it is vital to consider the baseline and then proceed to perform the measurement at systematic intervals to confirm whether the intervention impact reflects deterioration or improvement in relation to the expected level of variation performance. It is also essential to measure the state of the application without the intervention having in mind the external potential causes of change witnessed in the measurement. This can be achieved using the statically process control (SPC), a method explores the difference between common variation which can be controlled also known as the (special cause variation)SPC employs control chart s which display boundaries for acceptable variation in a process and application of interoperability.

Lean and Six Sigma Tools

The current and the future work flow state will be identified by both the lean and six sigma quality improvements methods in relation to the project application of interoperability. This will be achieved by the application of the following tools A3 Thinking,PDSA Cycles and DMAIC (Celenza 2017) .Lean refers to the culture and practice of relentless waste elimination to ensure that every services provided are of high quality, available and safe at the required time and delivered at a friendly cost. Every lean improvement always begin with the A3 Thinking since it is a methodological approach solving problems in the existing applications. The A3 thinking method enables the interoperability application to achieve a sensible, concise overview of the future and current operations and foretell precisely the occurrence of potential bottle necks in the interoperability project application. This is the reason the A3 Thinking as problem solving method has achieved popularity without its original inception field especially in healthcare and in interoperability application.

Conclusion

Several quality improvement analysis tools were reviewed in the relation to the interoperability application :six sigma,lean and ISO 9000 were used since many professionals in the healthcare use these quality improvement tools .The ISO 9000 was selected as the major and universal quality improvement tool in assessing the interoperability block chain application since its principles are similar to the Joint Commission principles. These principles focus on the importance of the need for great healthcare leadership and stakeholder’s interests. The tool approaches focuses on factual decision making standards that would assist the clinicians to make informed decision when handling the re hospitalized diabetic, substance abusers and mental health patients and in developing the care plans. The ISO 9000 tool predictor and criterion variables on the perception of the interoperability block chain application indicated that careful and close attention should be given to quality management in the implementation and operation of the block chain interoperability application. Implementation of quality standard data measures as exposed by the tool would also assist the clinician in making scientific informed decisions when handling these patients .
