Creating Action Plan

A solution to the Issue

The theoretical framework used for the study was ecological systems theory. According to this theory, different environmental factors and everyday encounters influence a person’s behavior. Moreover, proximal factors such as peer and family influence may trigger an individual to get exposed to unwanted conditions. The theoretical framework was necessary for explaining how substance and alcohol abuse relate to mental disorders. Nevertheless, the data from the organization indicated that the dual-diagnosis program was not fully implemented. Thus, there were fewer statistics on patients seeking treatment for co-occurring disorders (Gateway Foundation, 2021). There was also unequal racial distribution of the persons seeking services at the organization. This info showed there were few sensitization programs by the agency.

Therefore, the organization should improve the issue by conducting community education in the centers’ areas. This approach will ensure more individuals embrace dual diagnosis and treatment of co-occurring disorders when they visit the institution. Additionally, the organization should improve its services by expanding to other areas within Delaware to reach more patients.

Action Plan

The steps to be followed in the action plan include planning where the priorities and objectives of the program will be discussed. This strategy will consist of improving the dual diagnosis program to reach more individuals. The stakeholders involved in this stage will be the administrative heads, service workers, and community leaders (Gateway Foundation, 2021). These partners will identify the affected persons in the community and develop effective intervention measures to reach them.

Moreover, the action plan will include the implementation phase, where the developed intervention measures will be applied to the population of interest (Stringer & Aragón, 2020). In this case, the implementation stage will involve educating the community about co-occurring disorders to ensure that persons visiting the treatment centers undergo dual diagnoses. Thus, the stakeholders in this phase will include the service personnel who will conduct the community reach-out programs. Similarly, the patients will be part of the stakeholders as they will receive the intervention.

The final stage in the action plan will be the reviewing phase, where the participants will assess the progress to determine whether the proposed objectives would have been achieved (Stringer & Aragón, 2020). The administration and the service workers will rely on patient data and evaluate the admission and recovery rates to ascertain the program’s success.

Action Plan Continuation

The action plan will be monitored by regular evaluation to ascertain whether it meets the set objectives and goals. This strategy will ensure the action plan continues to serve its purpose of reaching more patients in the community (Stringer & Aragón, 2020). Furthermore, the assessment will include evaluating how the target population will respond to the intervention to make necessary adjustments. The administration and the service personnel will be responsible for ensuring continuity of the program after every six months.
