Module Assessment
Anticipatory care has been explained in the literature as approaches, which aim to prevent the development of long-term conditions as well as supporting patients to self-manage their condition.
In an essay of 3000 words and from your own clinical perspective, critically evaluate how self-management can support patients to live with their long term condition. Within this you should demonstrate knowledge of the module theory.
How will you demonstrate this in relation to health promotion?

The marker, in relation to the following assessment criteria, will provide feedback comments:
The structure of the essay which includes logical development of the topic, fluency, presentation and referencing

Extent to which the essay demonstrates analysis of relevant and contemporary literature to generate a logical argument
/50
Extent to which the content demonstrates knowledge and understanding of the subject area
/30
Extent to which the essay considers application of knowledge/relevant theory to support the argument
/10 .
NB The essay should be addressed from the perspective of the named award being studied (i.e. Nursing).
The pass mark for this module is 40%.
Essay format:
Use Times New Roman,Calibri, Arial font or similar in size 12 point
Use 1.5 or 2 line spacing, with at least 2.5cm side margins
Number your pages.
Include front sheet (attached).
