My initial reactions and observations of this case is seeing that both of the patients parents have health issues which would unfortunately mean the patient is even more prone to health issues. I can see that he has been diagnosed with sickle cell anemia as a kid due to both of his parents having it. This could be why he is not feeling well and having these symptoms, but then again I do not know too much about sickle cell anemia. I do know that it causes the red blood cells to have an abnormal shape which causes them to get stuck in your vessels and I also know that both parents need to have the disease in order for it to get passed down to your children. With Mr. Smith smoking cigarettes and drinking alcohol, that is not benefitting his health either. Mr. Smith’s ulcer on the medial side of his ankle could be caused from his sickle cell disease because the disease causing the red blood cells to get stuck in your small blood vessels leads to the reduction of oxygen getting to your skin. I would describe inflammation as your body fighting off infections or injuries. I can recognize that Mr. Smith has an inflammatory response due to him having pain and him having a high fever since those both can be signs of an inflammatory response. No, inflammation is not always bad since its part of your body fighting off foreign invaders and injury to keep your body healthy. I am not sure of any white cell diseases that reduces our inflammatory response. I know that allergies are your body reacting to usually harmless things in the environment like pollen or pet hair. I also know that allergies can be mild or life threatening. I think that a penicillin and iodine allergy is a type 1 hypersensitivity because working at a hospital I have seen patients that were allergic to iodine and it had only caused a rash. A malignant tumor is a cancerous tumor and is likely to spread and benign tumors are non cancerous growths and will not spread.

What are your initial reactions and observations of the case?                                                                     My initial thoughts and observations about this case are that Mr. Smith’s signs and symptoms are associated with his sickle cell anemia disorder. Also, finding out if he actually has a fever would be very helpful. With having the lesion on his ankle that is hot, red, and secreting purulent means his body is fighting an infection. Also, his father has hypertension, so Mr. Smith is at risk for developing hypertension as well. This could explain his Tachycardia.

Mr. Smith was diagnosed with sickle cell anemia. He has an ulcer in the medial side of his ankle. How would the condition of the red cells contribute to what the patient suffers from?                                With sickle cell anemia red blood cells die off early and are sickle shaped. This can cause blockage which makes for poor blood circulation. Without rich oxygenated red blood cells wounds take longer to heal, and the wounds are prone to infections.

How would you describe inflammation? How do you recognize that Mr. Smith has an inflammatory response? Is inflammation always bad? Do you know of any white cell disease that reduces our inflammatory response?                                                                                                                        Inflammation is a process that the body begins after an injury to slow blood flow down to that area. The body is trying to protect itself from bacteria and bleeding out. Mr. Smith’s lesion is red, swollen, hot, and secreting purulent which are all signs that Mr. Smith is having an inflammatory response. Inflammation is not always bad. It is our bodies’ way of trying to protect it from any further damage. Crohn’s disease and Rheumatoid arthritis are both white cell diseases that reduce inflammatory responses.

Mr. Smith is allergic to penicillin and iodine. What do you know about allergies? What type of hypersensitivity is it?                                                                                                                                             An allergy is when your immune system reacts abnormally to a foreign substance. You can have drug, food, and contact dermatitis allergies. These are hypersensitive reactions. There are 4 types of hypersensitivity, I, II, III, and IV. Hypersensitivity I is the worst, and this can be an anaphylactic shock reaction. 

Mr. Smith’s mother had a right radical mastectomy due to breast cancer. How would you define a malignant tumor? How would you differentiate a cancerous tumor from a benign tumor?                             A malignant tumor grows rapidly, has a loss of differentiation, and lacks a capsule. They also invade blood vessels, lymphatics, and surrounding tissue and organs. They also have the ability to metastasize. A benign tumor is usually well encapsulated and differentiated. They do not invade or spread to distant locations. When feeling a tumor you should be able to tell if it is freely moving and somewhat soft. This is a sign that the tumor is benign.
