Carl is admitted to the hospital with a fractured hip sustained from a fall when he got up during the night to check on Evelyn who was wandering in the house. Before his injury he cared for Evelyn, who suffers from advancing Alzheimer’s disease. While he is hospitalized, Evelyn stays with a niece who lives 100 miles away, but this cannot be a permanent situation because their niece is also in frail health. Carl and Evelyn’s children live across the country and are very involved in their careers. He is concerned about who will care for Evelyn and how he will manage when he is discharged. What approach would be the best to take in planning Carl’s care?
