Prioritise one nursing-relevant strategy.b. For your chosen strategy, draw upon available evidence to justify solutions orpossible strategies to minimise the risk of recurrence of the incident.Structure of the ReportStructure your report under these headings and include the information below.• Title: Identify the incident you have analysed.• Introduction (150 words)Clearly state what the clinical incident is, define what root cause analysis is, why usingit, what methods you are using (such as Checklist Flip), and a brief overview of what youwill address in this report.• Root cause analysis (350 words)Complete root cause analysis process that identifies possible root causes of theincident. The process is two-fold: First, develop a flow diagram to show the key events2 | P a g ecrucial to understanding the incident using a maximum of 6 boxes; Then describe andcategorise cause/s and/or contributing factors using the provided template.• Potential intervention (350 words)Prioritise one nursing-relevant strategy that is likely to result in meaningfulimprovement, which could be implemented within the setting where the incidentoccurred. Justify your choice of strategy.• The evidence base for intervention (500 words)For your chosen strategy, locate the best available evidence-based resources. Use thisevidence to support and discuss the effectiveness of the suggested interventiongenerally in health care.• Conclusion (150 words)Draw logical and insightful conclusions about the incident and strategies to prevent itsrecurrence.ReferencesA minimum of 10 references is required to support your arguments. These references should bedrawn from peer-reviewed discussion or research papers, systematic reviews, evidence-basedguidelines, or evidence-based systems and processes published within the last 10 years. Try toreduce the number of references (a maximum of five) from websites. If you do, ensure they are fromreputable websites, such as those ending with gov. or org. Non- or low-evidence-based sites, such asWikipedia, MyDr, Ausmed, Health Direct, or Betterhealth, are not acceptable to use as supportingevidence. Textbooks are not suitable sources to support your nursing-relevant strategy. Your chosencoroner’s report is included in the number of references (you need to use in-text referencing foryour chosen coroner’s report ONCE).APA 7th reference style must be used.For your reference list:• Start on a new page• Consistency, such as the format of DOI, capital, or lower cases• Alphabetical order• Ensure all references that appear in the report are listed in your reference listAdditional requirements• Have the first line of each paragraph indented using 1.27 cm.• The word limits in each section are a guide only.• Headings should be used.• Submit assignments as Word documents. PDF/Note pad assignments will not be accepted.No handwritten assignments will be accepted.• Include a Header with your name and student ID number and a Footer with the pagenumber.• Use size 12 Times New Roman, Arial, or Calibri font throughout the assignment with doublespace ONLY, align to the left.• The word limit 1500 includes headings, in-text referencing, diagram, and RCA table (onlyexcluding reference list). The word limit of 1350 to 1650 (+/- 10%) is acceptable. The markerwill stop the marking when the word limit reaches 1650. No marks will be taken for wordsfewer than 1350 but penalised for the relevant sections due to insufficient information.3 | P a g e• Insert word count (excluding reference list) into the required cover sheet.• DO submit your report to draft Turnitin to ensure academic integrity is maintained beforethe final submission. With the Turnitin report, it is fine that most references add significantlyto the overall similarity percentage. But ensure anything else highlighted is cited properly.• Submit your extension BEFORE the due date ONLY if needed with appropriate documents tosupport through special consideration. Items submitted after the due date without anapproved extension will be subject to the 5%-mark deduction per day late.Refer to the rubric for specific criteria and awarded grades for each section.
