During your 18 hours with a nurse manager, the focus of your experience should be directed toward the following:
Identify the leadership style of the unit manager you are shadowing. Did the person’s style change based upon different situations you saw him or her in? Did the style(s) seem appropriate for the situation? Might a different approach have been appropriate?
Identify how patient safety and staff skills or mix affect staffing assignments.
What are the staffing requirements for the unit? How was the mix decided upon? Who has the final say in determining this mix?
Explain the skills and understanding needed for how to staff a unit appropriately. How is unit staffing adjusted for higher or lower than expected patient census or increases and decreases in patient acuity?
Categorize the different ways patient satisfaction is measured and shared with staff. What changes are initiated as a result of this measurement?

Evaluate how the unit manager responds to changing patient safety, acuity, and census levels. Is there an acuity model used to measure patient acuity? You will also spend 6 hours in The Quality Department and discussing this role as it pertains to nursing. You are not to follow your immediate supervisor or unit manager at your place of employment. You can follow someone who works where you do, but not someone who is your immediate supervisor. Your goal is to learn about how a hospital manages and acquires quality information, in addition to how quality affects reimbursement.
During your 6 hours in the Quality Department, the focus of your experience should be directed toward the following areas:
How is Quality tied to reimbursement in this organization?
Compare and contrast the role of the Risk Management Department and the Quality Department. How does the Risk Management staff interact/work with the Quality Management staff?
What safety initiatives have been put into place? How have these initiatives impacted nursing and nursing care?
What is benchmarking and how is used by this organization?
What is the quality improvement approach used in this organization, how is it measured, and what tools are used?
How has managed care impacted nursing care at this organization?
How have nursing and nurses been affected by reimbursement, such as in waste reduction and staffing levels?
Is it important at this organization for nurses to understand how care is paid for and why?
How do professional nurses participate in the Quality Improvement process and provide examples?
How does this organization view incident reporting? What are the advantages of having staff complete incident reports? Do nurses at this organization fear completing incident reports?
How does continuum of care play a role in reducing costs in healthcare
How do patient education, promotion, and prevention improve quality of care and decrease costs?
Be advised, the above questions may be “politically correct” answers, as many organizations are unwilling to share such information with those they consider not on a “need-to-know” basis. Try to determine if what you are being told is such an answer by such things as body language, hesitation in speaking, and poor eye contact. However, do NOT challenge what you are told, as we, are guests in the organization.
