Part A: In a voice recorded PowerPoint presentation, you will analyse the communication processes occurring between the patient, family and nurse in the given case study based on your developed knowledge and understanding of communications frameworks and concepts of care covered in the course. You will identify and discuss the ineffective verbal and nonverbal communication evident in this scenario. You will then apply the effective communication framework as learned, to contribute in achieving therapeutic communication for patient and family. Integrate a SOAP note for the non-verbal inter-professional communication and a verbal clinical handover (SBAR format) on how you would communicate this in a nursing handover at the end of your shift, as per intra-professional communication. Your slide presentation must include the following: 1. An introduction: – Introduce yourself to the reader/listener – Provide an overview of the main points that your presentation is going to provide 2. In the body of your presentation: – Provide a discussion of each of the components in your presentation: i. Your analysis of the ineffective communication used by the nurse, patient and family in the case study. ii. Application of communication strategies, skills and approaches to achieve both effective and therapeutic communications, inclusive of discussion on caring conversations and cultural considerations to support the loss and grief of the patient. iii. The SOAP note that documents the ineffective communication. You will include in your presentation a slide of a documented SOAP note of the information exchange between yourself and patient and family, as part of written communication in healthcare. iv. The SBAR approach for clinical handover for consistency in therapeutic communication. You will provide an orally presented clinical handover using the SBAR format for intra-professional communication between nurses to ensure consistency of therapeutic communication is maintained. Provide in-text references in your slides to support your information and/or specific examples. 3. In your conclusion: – Provide a summation on how written and verbal professional communication contributes to patient safety and quality in nursing practice. Finish with a powerful impact statement. PART B: Portfolio Task – Using the first 2 stages of Gibbs Reflective Cycle, provide a reflective paragraph (in your Portfolio) about the resilience and self-care required for maintaining effective and therapeutic communications relative to the case study (below). Case Study Mrs Aadra Begum is an 84-year-old female who has recently been placed into an aged care facility in Toowoomba. She has been placed in the facility as she lives on her own and has been deemed unsafe to mobilise at home without some supervision and assistance to complete her activities of daily living. She is a widow of 3 years, still grieving the loss of her husband who cared for her in Bangladesh. Mrs Begum’s daughter relocated her to Australia in 2018 to care for her, in her own home. However, this has been difficult as her daughter travels interstate frequently with her job. Mrs Begum is very upset about leaving her daughter’s home and family after migrating to Australia. Every day she asks staff, “Please tell me when can I go back to my daughter?” You are the student nurse caring for Mrs Begum and you respond to a call from her nurse-call button. Mrs Begum’s daughter and family are visiting and they start to question you on why Mrs Begum has been told she “can go home this weekend as she is getting better” and “that the nurse told her today that she can go home.” You attempt to explain this is incorrect, when Mrs Begum interrupts you and states she was told this morning when being escorted to the bathroom “on my own, with no assistance needed”. Mrs Begum states you then praised her, for “doing so well on your own” and “completing all your morning cares and brushing your hair with no help at all.” You realise there has been a misunderstanding of your positive praise for Mrs Begum and you move towards where she is sitting on the side of her bed. You lean over closer to face her speaking slowly and loudly: “No, I didn’t Mrs Begum, you have that mixed up. I was just praising you for being very good on your own, not needing my help with your cares today.” With that, Mrs Begum’s daughter stands up, moves closer to you and states loudly, “Don’t you speak to my mother like that! She is not deaf and that was very disrespectful the way you spoke to her just now.” You step back from Mrs Begum’s daughter quickly, startled by her close presence. You then turn to speak to Mrs Begum who is still sitting on the bed. “Sorry Mrs Begum, but you need to tell your family the truth. I didn’t say you were going home. You have misunderstood my praise for you that I was suggesting that you are ready to go home today. This is not the case” You then leave the bedspace and speak to your colleagues
