II. Completing a “fish diagram”
Situation: You are diving on New Scotland Avenue heading east toward downtown Albay on a snowy and slick night. Your telephone rings and distracts you somewhat. The light at New Scotland Ave and South Manning Boulevard changes more quickly that you expect and you try to stop. Your tires are old and slide as you attempt to brake to stop short of the intersection. Your attempt is not successful and you move into the intersection and strike a car that is attempting to go straight onto S. Manning Blvd. No one is injured but this shakes you up. As you begin to consider the many factors that led to this accident, you decide to use the fishbone diagram.
Directions: draw a diagram and include at least four possible causes (bones) with factors listed underneath. The incident is a motor vehicle accident. Please prepare the diagram that outlines some of the possible conditions that contributed to this accident. (15 pts)
