Question #1 Based on your community observations in week 2, what was the most important data point? What finding was most surprising to you?

I think the thing that most impacted me as I traveled through the community was the uneven access to services, especially healthcare. The city of Flagstaff does work hard to provide services and improve equity (including health equity) through a variety of means, but it is clear that this only addresses part of the problem. Being a long term resident, I know that access to healthcare is extremely difficult for even those who have good insurance—many physicians, specialists, mental healthcare professionals—do not take new patients, as they are overloaded already. What about those who are uninsured, underinsured, or on state exchange-based insurance that is designed more for catastrophic care? Therefore, what I see as I travel through the city is that healthcare facilities are concentrated almost entirely downtown or near the hospital (just north of downtown). Outside of that, there are very few options. The city does have an excellent public transportation system that is inexpensive and has many stops throughout the city, but again, depending on circumstances it may not matter—it might not be possible to see a healthcare professional. There are a few free or sliding-scale clinics, as well as one catering to Native Americans, but these are few and far between. I have the luxury of good insurance and a reliable vehicle, but it’s clear that my city can continue to work on making healthcare access better for all citizens, regardless of ability to travel or ability to pay (e.g. insurance coverage).

The thing that was the most surprising to me was the cost of transportation within the city. $2 per person each way within city limits and $5 per person each way to go outside city limits. That is kinda extreme in my opinion. Our big metro transportation system is only $1.50 per person per trip and if you are disabled you get a discount. To provide a service for the people of your city but to charge them up to $4 in city and $10 outside the city is excessive.
Patients need to be able to rely on transportation that will not break the bank, so to say, and that will get them where they need to go. Some of our elderly rely on this transportation and they are on a fixed income and having to fork out this money on a regular basis can be difficult for them. It can mean having to choose from something important like medication and getting somewhere.
I know that gas prices play an important role in the cost of transportation, and that the costs will most likely just continue to rise. But, there has to be something that we as a state can do.

Nursing interventions are integrated into a multidisciplinary plan of care in the emergency practice setting from the moment they walk into the ER. Where I practice, a paramedic greets them and does a quick “COVID” screening. Then we determine if they go to a respiratory or non-respiratory area. Then The nurses receive the patient and hook them up to monitors, do IV etc. Then The MD/PA/NP come see patient and according to orders, RT, XR, CT, Ultrasound, etc. come see the patient.
So everyone comes into the patient care, we all do our part and keep good communication with each other to achieve the best possible outcomes when caring for our patients. According to Hanakova & Potmesil (2014) communication conducted by a multi-disciplinary team, members should employ a breadth of comprehensive care behaviours that incorporate a responsive and honest method of communication.

In my current position I work concurrently with providers of all levels to ensure patients (Injured Workers) receive complete and necessary care. The current title I hold is “Nurse Care Advocate.”. This position requires the use of nursing specific knowledge daily, it is the nurse who makes in initial contact with the person and assesses the injury and treatment plan. Gaps in the treatment plan are identified at this point, these gaps are then addressed by the nurse which requires a level of advocation, as the payor does not possess the knowledge to determine the type and level of care needed. The nurse care advocate position was created to ensure that the person does not get lost in the sometimes-complicated webs of the healthcare system, and to prevent over utilization of healthcare resources. The healthcare system is not designed to put the patients’ needs first, the nurse is positioned to advocate and evaluate these specific needs and bring those to the team as an extension of the patient (Sherwood & Barnsteiner, 2017).
The Swiss cheese model of the current healthcare system is my reason for interest in nurse care management. I have witnessed countless patients fall through the cracks and have poor outcomes, or overuse of precious healthcare resources due to lack of provider communication. Care management is a position in which the nurse can help people navigate the healthcare system, as well as heal faster and more efficiently. The nurse care manager can tie the healthcare team together through effective communication, one of the necessary characteristics of effective interprofessional communication (Sherwood & Barsteiner,2017).
