PLUS population is aging and in fact in 2019 those that are 65 years old are more than age five and below (Landers et al, 2016). Modern medicine has increased a person’s life span, but this means that a person is living with multiple chronic condition such as CHF, DM, Dementia, and all other diseases (Landers et al, 2016). They may also be saddled with functional impairment that the person may not be able to complete ADLs, IADLs or be able to manage their homes. On top of that people are having less children or their children has move away from home to other states for job opportunities living their parents managing their home and health by themselves. One of the most important tasks of the US health care system is to reduce health care cost and ensure that older population with multiple chronic condition can live as independently as possible. Confronting this issue may require maximizing the potential of home health-based health care. Things that are motivating health care to transitions to home health is cost, the need to improve health care delivery system and patient satisfaction (Landers et al, 2016). As discussed, US health care cost is ballooning and with more people entering Medicare it is the best interest of everyone to control cost. Patient that qualifies for Medicare has also multiple chronic condition. Delivering health care in the most efficient situation is of outmost importance. Medicare programs have been advocating for “Triple Aim” which is better patient experience, better population health and decrease the cost of health care (Landers et al, 2016). Program such as binding fee for service to quality and value of service has started. The aim is better service thereby reducing cost. As patient becomes more involved in their health care, majority prefers to stay at their homes for as long as possible instead of a facility. Knowing patient’s preference health care has shifted to home based care.
