Practical Assessment
The Practical Assessment is a set of tasks that must be completed in an industry workplace, or in a simulated industry environment with conditions similar to that of a real workplace. To be assessed for this unit of competency, you must demonstrate your skills and knowledge to identify, assess and control safety risks in the workplace. The Practical Assessments in this workbook include: Case Study Assessment Includes detailed scenarios and simulated environments, providing all necessary information required to complete relevant tasks and activities. These are based on the simulated business, The Continent Hotel. Workplace Assessment A series of tasks assessing the candidate’s practical knowledge and skills relevant to the unit of competency. This includes the candidate completing workplace documents or similar as evidence of competent performance. Workplace Practical Observation A set of assessment tasks where the candidate must demonstrate practical skills relevant to the unit of competency. These skills are to be demonstrated while being observed by the assessor. IMPORTANT! All signatures/initials in your submissions, including yours, must be handwritten and dated. Case Study 1
It is your first week as a kitchenhand at La Bellissima, the Italian restaurant at The Continent Hotel. One of your main duties include keeping the kitchen clean. La Bellissima’s kitchen has an island kitchen setup where the dishwashing station and the inventory storage sit together. While cleaning the benches, you notice the following: Oven cleaner container and detergent has been left off below the inventory shelves.Splatters of oil across the meal cooking station and the dishwashing station. You learn from your induction that any hazard you find in the kitchen must be reported and actioned immediately. You report this to your supervisor. They ask you to complete the necessary forms. Case Study 2
You learned that Carl, the kitchenhand from the closing shift, usually forgets to keep the cleaning agents in the proper storage. Carl has been reminded by his colleagues several times since last week to correctly store the cleaning agents and equipment before he closes the kitchen. You notice that there are no signages in the kitchen informing staff about this. However, staff are informed during induction which cabinets these cleaning agents and tools are to be kept after use. ***** It is a busy week at La Bellissima. Bulk of food items and other supplies are being delivered multiple times this week. You and other kitchenhands need to move around foods items and supplies in the storage to follow the ‘first-in, first-out’ policy of the restaurant. Staff are given rest breaks however the kitchen is currently understaffed due to some of them taking sick leave. Your arms and lower back feel sore in the last few days due to this repeated task. An upcoming long weekend and public holiday expects the same busyness in the restaurant. Complete the task on the page that follow. |

This workplace assessment requires you to identify hazards, conduct risk assessment, and eliminate or control the risks in your workplace. This assessment is divided into three tasks: Task 1: Identify Workplace HazardsTask 2: Conduct Risk AssessmentTask 3: Control Workplace Risks You are required to complete the assessment tasks in an operational business environment, or in a simulated industry environment with conditions similar to that of a workplace. Each task comes with a set of instructions. You are to follow these instructions to complete the assessment. Each task will require you to: Submit completed workplace templates and/or any required documentation; andDemonstrate task requirements while being observed by the assessor. Some tasks may include both requirements. These will serve as evidence of your task completion. Before starting this assessment, your assessor will discuss with you these instructions, resources, and guidance for satisfactorily completing the tasks. Task 1: Identify Workplace Hazards

While being observed by the assessor, work with at least two other personnel to identify and document hazards in your workplace. STEPS TO TAKE Access the following workplace documents:Policies and procedures for hazard identification and recordkeepingWork health and safety (WHS) planCopies of current WHS reports: Injury or illness registersRisk registers Reports must be from the last five years from the date of your assessment. Review the WHS plan to identify tasks or work conditions that can cause harm in the workplace. Work with at least two other personnel from your workplace to identify potential OR actual workplace hazards: Personnel can include your supervisor, other peers and colleagues, or any WHS committee members or representatives from the organisation. Review current WHS reports to identify existing workplace hazards.Conduct a safety inspection in all relevant areas of your work. Identify at least one actual hazard OR at least one potential hazard from each of the following areas in the workplace: Physical environment (i.e., electrical items, workplace design, hot and cold conditions) Plant (i.e., machinery, equipment, appliance, tools) Work practice (i.e., manual tasks, rosters and breaks, standard operating procedures for work-related tasks)Security issues (i.e., customer behaviour, managing cash) Write these in the Description of Hazard section in the Hazard Report Form. Task 2: Conduct Risk Assessment

While being observed by the assessor, work with at least two other personnel to assess the risks posed by hazards identified in the workplace. You will document this in the risk register template. These must be the same personnel who worked with you in Workplace Assessment Task 1. STEPS TO TAKE Access the following documents:Policies and procedures for risk assessment and recordkeepingWHS reports relevant to the hazards identified from Workplace Assessment Task 1 These can include existing reports you reviewed for the previous tasks, and other reports related to the new hazards you found. Hazard Report Form completed from Workplace Assessment Task 1Use your organisation’s template for conducting risk assessments. You may also use the Risk Register Template provided at The Continent Hotel site. Work with the two other personnel to systematically assess the risks from your workplace. Document all your findings in the Risk Register Template: Record ALL of the potential and/or actual hazards from the Hazard Report Form.Identify at least one risk that may be associated with each of the hazards identified. Indicate the type of each risk you identified, e.g., slips, trips, and falls (physical risks); exposure to hazardous chemicals (chemical risks) Describe the exposure of people or things in the workplace to the hazards identified. Determine the likelihood and consequence of each risk from occurring. Refer to the Likelihood and Consequence Scales in the Risk Register Template. For each hazard identified, collect at least two types of evidence to assist you in assessing the level of risk of each hazard. Your WHS evidence portfolio can include two or more of the following:Photos or videosRelevant WHS reports from the workplaceConsultation with other employees One type of evidence can be used for more than one identified risk; however, it must be clear in your submission. |
Assess the level of each risk you identified based on the evidence collected, the likelihood and its consequence. Refer to the Risk Matrix in the Risk Register Template. Include the risk level score in your response, e.g., a risk that is ‘Likely (4)’ to occur, with a ‘Major 3’ consequence has an overall risk level of 12. Discuss with the two personnel at least one recommended control measure to minimise or eliminate each identified risk. Identify the person/s responsible for implementing each recommended control, including yourself.Have the two other personnel sign the third-party declaration at the end of the template. Their signatures must be handwritten. Store the copy of the completed Risk Register Template and WHS evidence portfolio. Follow your organisation’s procedures for this. Document your implementation in the Risk Treatment Action Plan. STEPS TO TAKE Review the Risk Register Template completed from Workplace Assessment Task 2.Use your organisation’s template for documenting the implementation of risk control measures. You may also use the Risk Treatment Action Plan provided at The Continent Hotel site. Populate the Risk Treatment Action Plan with the following information from the Risk Register Template:Associated risks for hazards from each of the following:Physical environmentPlantWork practiceSecurity issues List each risk in order of priority based on the Risk Register Template. Risk with the highest risk level score will be listed first in the priority.
