Reflecting on your readings in this week’s learning activities, which particular topic or issue resonates the most with you and why? What big questions remain unanswered?
In this week’s readings the article ‘Framing change for high-value healthcare systems’ was very interesting. So many issues in our health care system have shown the need for reform. Few providers are in place to deliver high-quality care. Different payers have developed pay based on performance reimbursement, which could change to payment for value and therefore, as the market changes, providers have to change also by designing new ways to improve the value of care to their patients. One way this is being accomplished is providers coming together to form medical groups which helps with expenses and gives more support to clinical and administrative functions. There are different levels of structure of a medical group, depending on which area needs the most attention. This makes the medical group a complex adaptive system. Medical groups will be more successful when they understand how a complex adaptive system works. According to Neese et al., “most clinicians are well schooled in “linear” change (2010, pg.24). Linear change uses previously acquired information used often in such areas as diagnostics, testing, and treatment. This change is based on factors of how the groups behave and how capable they are. Change can only occur if all members of the group take part in the change. The author also talked about interdisciplinary teams. All members of the group attend and shares their outlook on patient outcomes (Neese et al., 2010). When I was a Hospice nurse, we had interdisciplinary team meetings on a weekly basis with the Medical Director. We were all updated on each patient’s care, such as a change in treatment, a change in medications, and if a pain medication had to be increased.
