Summary of strengths:

During my clinical training and educational experience, I have cultivated robust basic skills in several primary care procedures. I possess proficiency in doing vision and hearing exams, administering wound care, executing Mini Mental Status Exams, evaluating gait and balance, using peak flow meters, performing laboratory and diagnostic testing, and conducting cerumen removal (Fuller & McCauley, 2023). These qualities demonstrate my proficiency in routine evaluations and protocols, necessitating a thorough comprehension of physiological systems and patient-centered treatment. My proficiency in interpreting fundamental diagnostic data and conducting comprehensive physical exams has enabled me to significantly contribute to clinical decision-making. I am well equipped to do health tests and analyze results to identify patients at risk. These areas of expertise will provide a solid foundation for my practicum, enabling me to do more intricate treatments with greater independence as I advance in my role as an AGPCNP.

 

Growth opportunities:

Although I possess a robust foundation in fundamental assessments and prevalent primary care procedures, I acknowledge multiple critical areas for professional development during this practicum. I possess minimal experience with operations like punch or excision biopsies, suturing, fluorescein eye staining, wart excision, and nail trephination. I require additional experience in managing acute conditions such as epistaxis and in interpreting pulmonary function tests and radiological findings (Angelow, 2021). These gaps offer a significant opportunity for me to improve my technical skills and clinical judgment with preceptor supervision. Enhancing these competencies will bolster my confidence and preparedness to deliver comprehensive care to various adult and geriatric populations. I am enthusiastic about applying theoretical knowledge to practical situations and actively seek supervision and feedback as I refine advanced techniques and develop into a proficient clinician.

1.      Goal: Enhance procedural competencies in prevalent dermatological and minor surgical methods.

    1. Objective: To see and help with a minimum of three punch or excision biopsies under supervision by the conclusion of the second month of the practicum.
    2. Objective: Execute wart removal and tick/skin tag excisions autonomously under preceptor supervision by week 10.
    3. Objective: Achieve the successful completion of a minimum of two suturing procedures on suitable patients during the practicum (Guido-Sanz et al., 2024).

2.      Goal: Improve diagnostic interpretation skills to facilitate clinical decision-making.

    1. Objective: Precisely interpret a minimum of five chest X-rays and five EKGs under the supervision of a preceptor by the midpoint of the practicum.
    2. Objective: Partner with the preceptor to analyze and interpret laboratory and diagnostic tests during weekly case evaluations.
    3. Objective: Finish a self-study module on radiology and pulmonary function test interpretation and implement this information in clinical situations by week 8 (Katz et al., 2020).

3.      Goal: Acquire expertise in specialized primary care evaluations for geriatric patients.

    1. Objective: Administer a minimum of five thorough Mini-Mental Status Exams and record cognitive patterns in older persons by week 6.
    2. Objective: Conduct three digital rectal examinations and three prostate examinations under the observation of a preceptor before the conclusion of the practicum.
    3. Objective: Conduct vision and hearing examinations regularly during adult wellness visits and accurately document findings in electronic medical records (Peacock et al., 2022).

4.      Goal: Enhance proficiency in handling acute, episodic conditions within the primary care environment.

    1. Objective: Execute nasal packing or foreign body extraction in a minimum of three patients under the supervision of a preceptor.
    2. Objective: Successfully execute a minimum of one epistaxis management scenario or simulated skill laboratory session with teacher assessment.
    3. Objective: Conduct weekly reviews of evidence-based guidelines on acute condition management and engage in discussions during clinical team meetings (Blodgett et al., 2025).

 

 

 

Conclusion:

This self-assessment has facilitated a critical reflection on my clinical competencies, allowing me to identify my strengths and areas for enhancement. I intend to utilize this information to cultivate fundamental procedural skills, enhance diagnostic confidence, and deliver safe, evidence-based care to adult and geriatric populations. With well-defined goals and objectives, I am optimally situated to enhance my learning experiences during my practicum and further develop into a proficient AGPCNP.
