Reviewing Action Plan
The action plan steps I intend to take in my program involve planning, implementation, and reviewing (Stringer & Aragón, 2020). The planning phase will include setting the objectives and priorities of the program. The service workers and the administration will be responsible for creating the program goals. Similarly, the implementation phase will include applying the intervention measures to the population of interest. In this case, the implementation will involve educating the community about co-occurring disorders to ensure that persons visiting the treatment centers undergo dual diagnoses. Thus, the stakeholders will include the patients, service workers, and administrative personnel. The final sequence in the action plan will be the reviewing phase. I will evaluate the program to determine whether there will be improvements in patient admissions and recovery rates.
The action plan will be implemented by conducting a community education program to enlighten the members about co-occurring disorders. Moreover, the approach will prompt more patients to embrace dual diagnosis when they visit the treatment centers. The action plan has its strength since it outlines a clear purpose and objective of addressing the prevalence of substance abuse and mental health disorders. However, the action plan would be strengthened if I identified the required resources to complete the project.
