Clinical Scenario –
Diabetic Neuropathy A rehabilitation clinic run by your hospital has tasked you with creating an information tool about diabetes as part of a set of preventative-measures they are planning. Targeting adults in their 50s and 60s, your patient information tool should explain appropriate elements of the normal structure and function of the cardiovascular and nervous systems as they relate to the condition, addressing the changes that can occur to allow readers to develop understanding of diabetic neuropathy associated with Type 2 Diabetes. You should outline possible factors leading to the development of diabetic neuropathy, identify steps that can be taken to lower the risk of developing this condition, and explain the links to other related health concerns prevalent in this population such as poor peripheral circulation and skin ulcers.

Addressing important concepts covered within HLT100 that are relevant to your scenario, your patient information tool should be written to directly address your chosen clinical scenario. The target audience is the specified patient group within your chosen scenario.

However, you may utilise numbered referencing for this task to aid reader understanding if you prefer. Please ensure that referencing style is consistent throughout your entire submission.
• Paraphrasing is essential for the understanding of your target audience.
• Images may be used but please ensure they are referenced. See this link to assist: images If you were going to use this image anywhere else, further copyright requirements may be necessary, but as you are just submitting it for marking, and you are only sharing it with your tutor, this format will be sufficient.
• A reference list should be included within your Patient Information Tool
