Throughout this course, my attention has been directed towards the demographic of African Americans afflicted with Type 2 diabetes within our country. By examining this particular cohort, I have successfully discerned distinct obstacles exclusive to their circumstances while also unearthing potential resolutions to these specific issues. I have also learned the significant value of speaking to a particular demographic that cannot express their needs and concerns. One of the motivating factors for my decision to engage in research and expand my knowledge is exploring how I, as an advanced practice nurse, may successfully advocate by utilizing federal and state laws.
Identified Problem
The Problem I have been able to narrow into regarding the African American population living with Type 2 diabetes is health literacy. In my research, I have found that the lack of adequate health literacy has contributed to poor management and preventative measures for my selected group of people (Wasserman et al., 2019). I have also discovered that the African American population is at increased risk for a greater prevalence of diabetes diagnosis and more complex complications (Haw et al., 2021). Due to these factors, there is an acute sense of urgency in trying to figure out ways to address this challenge of health literacy among this selected group of people in our Nation.
Proposed State Legislation
The proposed state law heavily relies on the Diabetes self-management education (DSME) program, as outlined by the Texas Department of State Health Services (n.d.). The development of this program was initiated in response to the increasing prevalence of diabetes among minority populations in the state of Texas. The implementation of legislation aimed at specifically addressing the disparity in access to health information for the African-American community in Texas, with the goal of providing them with understandable and culturally relevant information. By engaging in this practice, significant attention would be placed. Healthcare practitioners should prioritize delivering comprehensive information to their patients at a level that ensures their understanding can be verified.
Proposed Federal Legislation
My Proposed federal legislation would follow the Affordable Care Act, which highlights initiatives around health literacy (Keim-Malpass et al., 2015); this would be specifically important in educating healthcare providers on the importance of health literacy and being able to identify patients and family members who may require assistance with comprehending the information provided to them. On a federal level, I propose implementing laws that would help regulate and govern healthcare regulatory agencies such as JCAHO to enforce annual training for all healthcare staff on health literacy among all patients. This is necessary as health literacy ” encompasses the social and intellectual skills that enable a person to acquire, analyze, and comprehend health information to make informed healthcare decisions regarding medical services and treatment for one’s life” (Asharani et al., 2021).
Questions about your Legislative Agenda:
Although the DSME program was created to provide information and skills to enable individuals to manage diabetes and similar conditions better, what programs are also in place to ensure the patient and their families understand the information provided? I would also like to know if the emphasis should be solely on the patient and their families or should some focus also be placed on ensuring that healthcare providers are verifying comprehension of such information.
My second question is directly in conjunction with the Affordable Care Act about what regulatory systems could be in place to ensure educational training is provided on an organizational level to every healthcare facility in this Nation instead of relying solely on systems such as JCAHO.
