Write an follow up PSYCH VISIT, chief complaint, , Psychiatric visit HPI, current Psych meds, exam, , diagnosis and recommended Plan for a 94years old Female currently residing at a skilled facility. She has dx of Insomnia, GAD, Adjustment disorder with depressed mood. She is currently on Sertraline 25mg for MDD, Hydroxizine 25mg BID, Remeron 7.5mg for appetite stimulant, and Melatonin 5mg at 3am for Insomnia. Patient said pill they give still does not help me fall asleep or stay asleep . I really have a big sleep problem. Appetite is ok and mood has improved since Sertraline dose was increased on 4/03/25. Plan is to start Trazodone 25mg at HS (Spefically 10pm) to manage insomnia
