Concept maps are a tool that provide a visual representation of information. Using concept mapping, show how the different agencies involved in health care, including social service agencies, interact, working to set out cohesive services in your community. Your concept map should answer the following questions: Thinking broadly, what is needed to ensure health services, including health promotion and disease prevention, follow a systems thinking model in your community? How might public health policy influence how different agencies impact the social determinants of health, thereby ensuring the systems thinking approach to services? To create a concept map, you can use tools such as Microsoft Word, Microsoft Powerpoint, MindMeister or LucidChart. You can choose your preferred tool. Keep in mind that, although you have free access to Word and PowerPoint as a student, other tools may charge a fee for use.
