Now that you have explored your professional values and ethics, it is important take it a
step further and design person-centered care self-management tools for your clients.

Preparation
For this assessment, you will identify a client in your practicum site. You will use
motivational interviewing techniques to discuss their readiness to address self-care
management of their health concerns. You will discuss current and emerging
technologies that may be helpful to the client to support self-care management. You will
then record a 5–7 minute video and include detailed speaker notes that explain the
outcome of your experience with the client in your practicum setting.

Instructions
This assessment will consist of two parts. The first part will start with selecting a client at
your practicum site. You will employ motivational interviewing techniques and strategies
to help the client develop SMART goals about how they intend to address the self-care
management of their health using current or emerging technologies. Then, you will
record a 5–7 minute video of yourself presenting your findings and create detailed
written speaker notes. It is advisable to create the speaker notes first, so that they can
act like a script for your video.

content should be clear and succinct. Use of AI is not permitted.
Written speaker notes: Submit detailed written speaker notes for the content of your
video.
Number of references: Cite a minimum of 2–4 sources of scholarly or professional
evidence that support your critique, reflection, and evaluation. Current source materials
are defined as no older than five years, unless it is a seminal work. Be sure to cite
evidence to support that your information is evidence-based. Make sure to include
references at the end of your speaker notes.
APA formatting: Resources and citations are formatted according to APA style.
