This assignment should contain the following separate components which can be uploaded in one document for submission (the timesheet can be uploaded in a separate file on blackboard also):

1. Completed attendance sheet demonstrating 30 hours contact and signed by person in authority with contact details included (student should develop their own template) (Pass/Fail)
2. Outline of the organization profile (Pass/Fail)
3. Learning outcomes identified by student and person in authority at placement (Pass/Fail)
4. 500 word reflective account of their experience with reference to how identified learning outcomes were achieved (include relevant reference list) (Pass/Fail)

Academic Presentation

• All assignments must be word-processed.
• All assignments must include a fully completed School’s “Submission Form for Assignments” to include: Student name, student ID no., Year of Intake, Module, Date of Submission, Assignment Title, Module Leader, Word Limit, Actual Word Count and Student’s signature.
• All assignments must include a list of contents.
• All assignments must have pages numbered sequentially.
• All assignments must use the Harvard Referencing System.
• In writing any assignment you must develop as fully as you can, the ideas or points identified.
• Undertake a spelling and grammar check on the final draft of your assignment. Read it carefully prior to submission.
