Mr. Benton visit was completed via telehealth video call using supervisor’s phone number. He is 34yr old, A&O X4, verbally compliant, denied pain or discomfort. Verbalized intact skin, well dressed while in the community. Consumer was communicated with his supervisor. He expressed proper behavior while in the community and respect other people’s right. Consumer expressed his desire is to start living with his family member. Consumer verbalized he has 4yr old child and has missed him so much because he stays in group home. Nurse educates consumer on importance of family support system as related to health condition. see rubric below
