Using the Centers for Disease Control (CDC) Adult Immunization Recommendations, review five patient records for Immunizations.

Select five individuals age >64 years of age from your clinical rotation. Review their EMR and/or query the person to update the EMR for CDC Recommended Immunizations appropriate for the age of the individual. For each missing immunization, please provide a rationale for why the immunization was not performed and whether that was appropriate. Indicate if the individual should be encouraged to now get the immunization. You may use a grid format. Please maintain HIPAA compliant documentation in your grid or paper and upload it as a Word document.
