Mental Health Outreach Program
“COVID Connection Commission” (CCC)

1. Have social workers visit clients in person in order to check on their mental health related to COVID
2. Use workers to help connect clients with therapists/families that haven’t been able to see one another due to COVID
3. Scheduling Zoom meetings and teaching clients to utilize Zoom in order to teach them how to stay connected
4. Scheduling in-person meetings with friends/families when possible to prevent clients from becoming socially isolated.
5. Organizing vaccination appointments for clients.
MISSION STATEMENT: To reach out to older adults with mental illness who have been impacted by the COVID-19 pandemic and remove/prevent isolation by keeping them connected to their families, friends, therapists, and other significant resources in the community.
