The patient safety problem/quality issue can be identified from several sources for example: NHS Improvement, CQC report, Ofsted report, problem/issues identified via DATIX, serious case review, research evidence, and common concerns raised in the media. The focus will be to address this problem/issue and provide a structured plan to support improvements through the service improvement report.
